The Quality Care Commission (CQC) has published its annual State of Care report, highlighting problems in social care.
The report claims the health and social care system is gridlocked.
According to the report, a large number of people are in hospital longer than they need to be due to a lack of available social care, and people waiting for social care support for too long.
The report also highlights inequalities across the social care system, saying: "It is vital that everyone, inclusively, has good quality care, and equal access, experience and outcomes from health and social care services."
The report called on social care providers to do more to make services more accessible, especially to people with different communication needs.
In regards to the overall state of health and social care, social care is performing better than other areas of the care sector - 83 per cent of adult social care services are rated good or outstanding and the report praises staff who are working relentlessly to keep people safe.
Social care service providers are struggling to recruit and retain staff to meet the needs of people in care. Staff are being drawn to industries with higher pay and better conditions.
Staff shortages in social care are worse than in the NHS - nine in 10 NHS leaders have warned of a social care workforce crisis in their area, and this is expected to get worse and have an effect on the NHS.
The State of Care report is published annually and and looks at trends, shares examples of good care and highlights where care needs to improve.
